- Provides Disability Assessments and Plans of Care (POC) with recommendations based upon expert experience, clinical assessments and peer-review, aimed to improve client health, living conditions, and insight for ongoing medical and financial planning
- Offers a unique care management approach for clients with traumatic brain injuries, debilitating mental health conditions, cognition and memory-impairments including dementia, severe physical and behavioral disabilities, those struggling from addiction, are considered to be dually-diagnosed OR family members in need of respite or general support services and care planning
- Determines needs for in-home safety equipment and recommendations for in-home safety modifications
- Promotes and seeks client placement within recreational, academic, vocational, social and art-based therapy or community programs as beneficial and appropriate
- Contract negotiations and enforcement for care provider, CNA, LPN, RN and other in-home care service agencies or individuals
- Time-tested, effective oversight and management of the "care team", including but not limited to, client physicians, therapists, care providers, companions, personal assistants, homemakers, friends, family and otherwise persons or entities beneficial to the POC
- Medical and financial auditing to ensure compliancy for short and long-term planning
- Crisis Management and Intervention
- Educational assistance, including advocacy and co-facilitation of IEP and other specialized support services for minors
- Health Insurance navigation, enrollment, appeals, disputes, compliance and general health insurance questions
- Money management, small accounting, tax prep and other support services available
LCMS provides oral and written testimony in probate court. This may include reports with recommendations, Plans of Care and budgets to assist the client, estate planning attorneys, the court and family members or interested parties of the client. LCMS acts as a neutral third-party among family members and parties in dispute over the care and planning of an individual. LCMS also provides supervised visitation when ordered by the court.
Highly-experienced in applying for, obtainment and management of Federal, State and community-based benefit programs. E.g. The Social Security Administration, Department on Aging, Department of Veterans Affairs, Department of Human Services and the Chicago Housing Authority.
